What is a J88 form used for?

Constable Mbali Masando of the Linden Police Station in Johannesburg explains that a J88 form is a key document recording medical evidence that will assist a prosecutor to obtain a conviction in an assault case.

This form is also critical when medical evidence is presented in a rape, attempted rape or sexual assault case.

Who can complete a J88?

The J88 form must be completed by a Doctor (or any other registered medical practitioner with forensic training) either at a private practise if the victim can afford to pay for it or at a state hospital or clinic where it will be done free of charge. The Doctor will indicate on the form where the victim sustained the injuries. It is important to photograph these injuries if possible.

Where can I get a J88 form?

Ask the police if they have any J88 forms. If they do not have any J88 forms, download the form from the Department of Justice and Constitutional Development website at www.justice.gov.za

How much time do I have to report an assault to the police?

Within three years after the incident.
